Description

Provides overall case management services to an assigned case load, coordinating plan of care with all relevant others. Responsible for completing required program participant reviews in a timely fashion, maintaining all records in compliance with State and Federal regulatory standards. Develops and implements habilitative plans for consumers receiving Waiver Services.

Requirements Bachelors degree from an accredited college with a major in human service. Valid driver's license, use of personal vehicle and automobile insurance (minimum of New York State's statutory requirements for personal liability) or equivalent accommodations is required.
