Description Practice Manager overseeing all Health Related Operations ( A16 A28) , will have county wide multi-site responsibilities. Responsible for the smooth, efficient and cost-effective operation of assigned clinical programs that serve a diverse patient population. In conjunction with and/or under the direction of the Assistant Executive Director, is responsible for all activities that improve performance on the triple aim of improved individual and population health, improved patient experience, and reduced cost. To guide the day to day aspects of satisfactory patient visits, workflow efficiency, quality of care and health outcomes. Ensuring adherence with all regulatory compliance, quality assurance plans and fiscal management oversight.

Bachelors degree and four (4) years administrative experience in a clinical, medical or rehabilitative program setting, a minimum of two (2) years of which were supervisory in nature; OR Masters degree and two (2) years administrative and supervisory experience in a clinical, medical or rehabilitative program setting. Valid NYS driver’s license with driving record acceptable to agency insurance carrier, use of a personal vehicle or equivalent accommodations, and willingness to travel for work is required. Practice Management Experience Strongly Preferred.
Demonstrated expertise in one or more of these areas:

  • Quality Improvement and Performance Improvement
  • Patient Centered Medical Home (PCMH)
  • Revenue Maximization/Billing Cycle Optimization
  • Care Management
  • Patient Access to Care
  • Health Information Technology (HIT)
  • Health Care Education, Training, or Coaching
  • Clinical background or experience working in clinical setting highly desired.
  • Performance Improvement experience, particularly in community health
  • Experience managing multiple simultaneous programs involving cross-functional teams within budgetary and schedule constraints, including addressing competing priorities
  • Experience directing internal and/or external teams, including resolving conflicting views
  • Experience effectively facilitating for teams and ability to manage team dynamics
  • Demonstrated skills and experience in negotiation, problem identification, and resolution
  • Strong teamwork, communication and interpersonal skills; ability to work effectively as a team member
  • Knowledge and understanding of QI approaches and how to use data and feedback for QI.
