Description Under the direction of the Director of Care Coordination and Behavioral Health Services, is responsible for the smooth and effective management of the Health Homes program, including administrative and clinical supervision of health home staff. Responsible for maintaining compliance in all clinical, quality assurance, financial management, policy/procedure development, outcomes management and productivity standards within Health Homes program. Will need to travel between Jamestown and Dunkirk.
Requirements

Masters degree in Social Work or recognized mental health discipline as defined by the New York State Office of Mental Health and licensed in the State of New York as an LMSW or LCSW required plus one (1) year experience working with individuals diagnosed with severe, persistent mental illness or complex medical issues and/or community social work and one (1) year proven clinical and administrative supervisory experience in a human services program.  Must possess Valid NYS driver’s license with driving record acceptable to agency insurance carrier or equivalent accommodations is required.
