Summary: The Hospice Evaluator is a Community Health Nurse or Registered Nurse whose responsibilities and functions are concerned with the care of the terminally ill patient admitted to the Hospice & Palliative Care Team.

Essential Responsibilities/Accountabilities:

The functions of the Hospice Evaluator include but are not necessarily limited to, the following:

  1. Assess patient and family needs and determines their eligibility for care in the Hospice program. 
  2. Establish the initial plan of care with the Interdisciplinary Group (IDG). 
  3. Participate in Interdisciplinary group meetings as assigned.
  4. Other duties, as assigned.
  1. Graduate of an accredited school of registered nursing and a valid NYS RN license required;or Bachelors degree in nursing or one year of home health care nursing experience. CRNH preferred.
  2. A minimum of one year of related experience and knowledge of Hospice regulations required.
  3. Must have an interest in and sensitivity for the care of terminally ill patients and their family.
  4. Ability to travel required.
  5. Must demonstrate solid interpersonal, organizational and time management skills.
  6. Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.
  7. With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
