Applicants Please Note:  This position is located in Rochester NY.  This position requires current Home Health Aide certification as confirmed by the NYS Dept of Health Home Health Care Registry at https://apps.health.ny.gov/professionals/home_care/registry/home.action .  Applicants with current NYS CNA certification will be considered. 

Applicants must have flexible part time availability to work up to 32 hrs per week (day shift one week, evening shift the alternate) and every other weekend.

SUMMARY: The Hospice Aide is a Certified Home Health Aide who provides the quality care necessary to maintain the patient’s physical and emotional well being in the inpatient hospice setting. The position assists with the physical and personal care of the hospice patient  and their activities of daily living, including assistance with meals, maintenance of safety and environmental upkeep. The Aide assists in maintaining a supportive and empathetic environment for patients and families.

Essential Responsibilities/Accountabilities

  • Follows Hospice Plan of Care for each individual patient as directed by the RN.
  • Observes patients’ conditions and reports any and all changes in patient condition to the RN.
  • Turns and repositions bedridden patients alone or with assistance.
  • Answers patient call lights.
  • Maintains a neat and clean patient area. Changes linens as needed.
  • Completes light housekeeping as necessary.
  • Feeds patients who are unable to feed themselves. Serves and collects meal trays.
  • Measures and records food and liquid intake/output and vital signs – reports any changes to professional staff.
  • Provides patients assistance with walking, exercising and moving in and out of bed if needed.
  • Provides assistance with personal care – bathing and grooming.
  • Gives regular attention to the patient’s comfort.
  • Offers support and comfort to families at the time of patient’s death.
  • Assists RN and other disciplines as it relates to health and non-health related tasks.
  • Stocks/stores medical supplies and linens.
  • Performs other functions as assigned by management.

High School Diploma or equivalent and NYS Home Health Aide certification required. C.N.A.s are welcome to apply but will be required to obtain HHA certification through Lifetime Care.  At least one year of C.N.A. or Certified Home Health Aide experience. An interest and sensitivity in caring for the terminally ill. Ability to read, write, understand and carry out directions/instructions and maintain simple records. Must respect patient confidentiality. Accomplishes duties of the job within allotted time. Work is thorough and neat.
