The RN, Senior - HC functions in an expanded role that combines direct nursing services with leadership responsibilities designed to assist in the management of the nursing department. This role may include assisting in orientation of new staff, role modeling, co-visiting to evaluate or demonstrate clinical skills, participating in quality assurance activities, identifying in service needs, designing inservice programs to address identified needs and participating in the evaluation of nursing staff. Essential Responsibilities/Accountabilities: 1. Assists supervisor to ensure that referrals for nursing service are appropriately assigned in a timely manner. 2. Participates in the orientation of new staff by conducting orientation classes, providing observation and co-visits, and assisting with case management. 3. Serves as resource person to agency staff in areas of clinical expertise and participates in development of special clinical programming. 4. Participates in the ongoing development of selected nursing staff through provision of assistance in case management, provision of co-visiting, and identification of inservice needs. 5. Participates in the identification, design and implementation of area inservice program. 6. Assists in the ongoing monitoring and evaluation of individual staff performance. 7. Practices according to Agency and community standards. 8. Represents the agency at community meetings as appropriate. 9. Provides direct service to selected patients including complex cases and maintains a reduced productivity. 10. Perform other job-related duties as requested. 11. Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures. 13. Regular and reliable attendance is required

Requirements

BSN preferred from accredited school of nursing. Requires a valid NYS license as a RN and at least 1 year experience as a CHN in a certified home health agency. Knowledge of current community health nursing practices. Must have and maintain a valid NYS driver’s license. Demonstrates strong interpersonal skills and demonstrates strong teamwork skills.
